Job Description
Primary responsibility for coordination and provision of direct consumer services
- Facilitate therapeutic and didactic groups in a creative manner
- Development, review and updating of Individualized Recovery Plans with consumer
- Maintain assigned caseload of approximately 18 consumers.
- Establish a therapeutic relationship with consumers and utilize counseling skills
- Provide leadership, support, advocacy, assessment, education, and referral to consumers
- Provide or coordinate delivery or linkage.
- Provide or coordinate Co-Occurring services, if so designated/needed.
- Supervise recreation, socialization, educational and other activities
- Follow Professional and Ethical Standard of Practice
- Demonstrate knowledge and understanding of clinical terminology, and interventions
- Contribute to program assessment and development
- Coordinate treatment with outside providers; Outreach consumer's residence as needed, maintaining an ongoing relationship and contact with staff/family
- Identify community resources for program and consumers

Case Manager
Qualifications
Candidate must possess a minimum of a bachelor's degree in a human services field (psychology, social work, counseling)
Candidate must also have at least one year experiencing providing Chemical Dependency programming and is ideally working toward CADC certification or has completed CADC/LCADC certification.
Job Description
Primary responsibility for coordination and provision of direct consumer services
Assist in the development of Chemical Dependency/Co-occuring services
- Facilitating Co occurring (mental health + substance abuse) groups
- Have knowledge of AA/NA programming and assist consumers with linkage to these services.
- Understand and implement the 12 step principles .
- Educate consumers about the interactions of drugs/alcohol, medications and symptoms
- Demonstrate knowledge of relapse triggers and relapse prevention methods
- Development, review and updating of Individualized Recovery Plans with consumer

This position is a split shift. 7am to 9pm and 3pm to 5pm.
Transport clients to and from their place of residence to Monmouth Partial Care facility.
